Agreement for Dusk-to-Dawn Outdoor Lighting

The undersigned (hereinafter called the "Applicant") hereby applies for membership in Co-Mo Electric Cooperative, Inc., (hereinafter called the "Cooperative"), upon the following terms and conditions:

1. The applicant will comply with and be bound by the provisions of the Bylaws of the Cooperative, and such rules and regulations as may from time to time be adopted by the Cooperative. The Bylaws can be viewed online at www.co-mo.coop.

2. The applicant requests that High Pressure Sodium dusk to dawn light(s) be installed (if applicable) by the Cooperative at the place(s) to be agreed upon by Applicant and Cooperative as noted below.

3. Applicant agrees to the following:
(a)To pay Cooperative for said service a monthly charge of each light installed or in accordance to our most current "Schedule of Charges". (b) If the dusk to dawn outdoor lighting is initially installed at the location, applicant agrees to receive and pay for said service for a period of ONE (1) year from date service is made available.
(c)If at any time, Cooperative modifies its standard dusk to dawn rate available to consumers in this classification, Cooperative may make a corresponding modification in the rate for service hereunder.

4. It is understood and agreed by and between the parties:
(a) Service is available only where Cooperative has secondary service available.
(b) Additional poles or equipment (excluding transformer) will be installed by Cooperative and charged in accordance to our most current "Schedule of Charges".
(c) All right-of-way clearing or trimming to be done by Applicant.
(d) Cooperative is sole owner of light(s) and related equipment and will maintain the equipment as required to render adequate service; Cooperative will make such repairs or replacements within a reasonable length of time after having been notified by Applicant.
(e) This agreement shall be subject to any rules and regulations hereafter adopted by Cooperative relating to such service.

INSTALL DUSK TO DAWN LIGHT ON POLE (i.e. Meter, Transformer, Main Line, Lift Pole). FACE IN DIRECTION. CONSUMER WILL TIE RIBBON ON POLE WITH KNOT FACING DIRECTION FOR LIGHT TO FACE.

LIGHT NUMBER(S) IS CURRENTLY INSTALLED AT LOCATION. APPLICANT AGREES TO PAY CHARGES AS STATED ABOVE IN PARAGRAPH 3.

APPLICANT DOES NOT WISH TO KEEP DUSK TO DAWN LIGHT(S).
